Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. Specific results are not guaranteed. It is recommended that you see a licensed physician or licensed health care professional for any physical or psychological ailment you may have. Reiki is not a substitute for medical or psychological diagnosis and treatment. Reiki practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and nothing said during the session should be construed as such.
While Reiki is not contraindicated to any known medical conditions, it is recommended to disclose your known medical conditions before the session should any medical emergency arise. It is also important to keep your Reiki practitioner updated on changes in your medical profile.

Disclosure Statement as Required Under SB-215
As a Complementary and Alternative Health Care Practitioner, I am not licensed, certified, or registered by the state of Colorado as a health care professional. I am not a licensed medical physician and do not diagnose, treat, or prescribe remedies for the treatment of disease. The services I perform, whether in person, by web conference, or by phone, are at all times restricted to complementary and alternative health care services intended for general well-being. I am prohibited from performing surgery or any invasive procedure, administering or prescribing x-ray radiation, prescribing prescription drugs, using general or spinal anesthetics, administering ionizing radioactive substances, using a laser device that punctures the skin, performing enemas/colonics unless board certified, practicing midwifery, practicing psychotherapy, performing spinal manipulation, practicing optometry, directly administering medical protocols to a pregnant woman or a person who has cancer, practicing dentistry, setting fractures, practicing massage therapy, providing a conventional medical disease diagnosis, or recommending the discontinuation of a course of care recommended by a health care professional.
I am also prohibited from treating children less than two years of age. In order to treat a child who is between 2-8 years of age, I must have a written, signed consent of the child’s parent or legal guardian.
As my client, you should discuss any recommendations I may provide with your Primary Care Physician, Obstetrician, Gynecologist, Oncologist, Cardiologist, Pediatrician, or Pediatric Health Care Provider, or other Board-Certified Physician. I recommend that you consult with your healthcare provider before receiving any complementary and alternative health care services. Please note that I do carry liability insurance applicable to any injury caused by an act or omission in providing complementary and alternative health care services. A copy of your signed disclosure statement will be kept on file for at least two years after the last date of service.
By scheduling and receiving a Reiki treatment and/or by enrolling in and participating in a Reiki class, you acknowledge and agree to the terms stated in this disclaimer. Thank you!

By participating in Primordial Sound Meditation or other meditation classes offered by Allison Martinez, I acknowledge that I am doing so voluntarily and at my own risk. I understand that meditation instruction is not a substitute for any medical or psychological treatments that may be recommended by healthcare professionals for any physical or psychological condition or complaint. I am fully responsible for my physical and mental well-being before, during, and after each class session. I understand that some class sessions may include breathing exercises, and I agree to participate only within my range of physical comfort. I hereby release (Your Name Company name) and all its instructors from any liability for injuries occurring now or in the future, during or after my participation in these exercises. I understand that any instruction provided during the course is solely for me personally and may not be appropriate for others. I agree to keep confidential any information shared during the course. I have read and understand this agreement and I hereby agree to release, indemnify and hold harmless Allison Martinez (also trading as peacefullybe.com) and its instructors from any and all claims, actions, suits or other forms of liability that may arise out of my participation in Primordial Sound Meditation or other meditation classes.
